GP Referral Scheme

What is the GP Referral Scheme?

The GP referral scheme enables your GP to prescribe a course of exercise or activity as part of the rehabilitation process following physical or mental illness.

The types of GP referral programme offered vary, typically lasting between 10 and 20 weeks. On your last visit a final assessment will be undertaken to show the positive results and encourage you to continue with your new healthy lifestyle.

Upon successful completion you can access the range of leisure services offered by any South Ribble leisure facility and your progress will be recorded by a qualified instructor from the Active Health Team.

How do I join the GP referral scheme?

The first step is to visit your GP. Following a successful referral from your GP, Practice Nurse or Consultant you will be asked to contact the Active Health Team (contact number supplied by your GP) to arrange your initial appointment with a qualified instructor.

Your appointment will include a simple fitness assessment (please wear comfortable clothing in which to exercise). The results of this and the reasons for your referral will form your personal fitness programme to help you on the road to a healthier lifestyle.

Completed a GP referral programme?

If you have successfully completed a programme following a referral from your GP, congratulations, you are now automatically eligible for a Passport to Leisure Card which offers discounts for the health and fitness suite and swimming pools operated by South Ribble Leisure.
